How an assessment works

From referral to report

Every assessment follows a consistent process and is reviewed by our Clinical Nurse Consultant, ensuring quality, clinical oversight and support for our nurses.

  1. Step 1

    Referral

    Send through your referral, including clinical needs and support requirements.

  2. Step 2

    Pre-assessment call

    We'll call to talk through the client's needs, funding and any reports required.

  3. Step 3

    In-home assessment

    We visit the client at home. Family or support workers can be there too.

  4. Step 4

    Report and recommendations

    A clear, clinically reviewed report with practical recommendations.

Our specialist Parkinson's disease nursing consultations are delivered by a Clinical Nurse Consultant with extensive experience in movement disorders, clinical leadership and the development of specialist nursing services in the public health system.

We partner with NDIS participants, Support at Home recipients, private clients and healthcare providers to deliver tailored nursing assessments and specialist clinical support.
